Forms/Paperwork
Paperwork will be completed during the evaluation/first session. It can also be completed prior to the initial consultation per request. This will include Health History, Consent to Treat, and Practice Policies. If you have any paperwork from your doctor or other practitioners you are working with that you would like me to be aware of, please bring them with to your initial appointment.
